You're Eligible If:
- You have traditional Medicare Part A and Part B (NOT Medicare Advantage or PACE)
- You have a clinician-confirmed dementia diagnosis (any type—Alzheimer's, vascular dementia, Lewy body, frontotemporal, mixed, etc.)
- You live at home in your own home or community setting.
- You live in a participating Bay Area county (Santa Clara, Santa Cruz, San Mateo, Alameda)
- Not on hospice care
